Manual FUE Hair Transplantation

Technological Advancements of the late 20th Century which further progressed in the 21st Century changed the world by altering the perception of an average consumer, worldwide. Information became readily available; people became accustomed to quick information, quick results, and instant gratification. Young men no longer wanted to wait till they’ve reached the complete state of baldness! They wanted to reverse the signs of balding from the early start, hence new demand was created.

 Surgeons needed to come up with a Method which would allow young men and women to restore their lost density in the early stages and still allow for further restorations to take place as their hair loss progressed. They further demanded a method with no visible scarring which would prevent telltale signs of a cosmetic procedure; they demanded most natural results with quickest recovery time, with minimal side effects.

 The Last Decade of medical advancements in the field of Hair Restoration paved way to FUE Method (Follicular Unit Extraction). Early FUE Method entailed MANUAL extraction of individual ‘Micro-Grafts’ from donor site by the use of micro-punches ranging from .6 to 1 mm in diameter. The grafts which were extracted were tremendously smaller compared to the ‘old’ plug method, and were even smaller in diameter compared to the FUT (Strip) Method, hence the technique became known as ‘neo-grafting method’ or ‘micro-grafting method’. The procedure itself is classified as Minimally Invasive or Non-surgical because the is no linear tissue removal from the donor site. FUE Method does not involve any suturing, prolonged recovery, post procedure discomfort, likelihood of adverse effects such as infection is minimal, and best of all there is NO LINEAR SCAR OR TELL-TALE SIGN of a transplant as a result of FUE Procedure.

Just when the public believed that ‘ideal’ form of hair transplantation was finally available, critics came up with numerous findings and claims that propagated against the Manual FUE.

  1. The Method is very Technique Dependent and senior, plastic surgeons that were set in their own ways were reluctant to learn the New Technique.
  2. The Method itself takes too long to complete ( 8 to 10) hours, however only a small number of grafts could be extracted and implanted (700 to 1400) maximum, per clinical session. For most people, these numbers were not sufficient to achieve decent restoration.
  3. Because the procedure time and the ‘surgeon’s time’ was prolonged, the cost of the procedure had doubled in comparison to the Conventional FUT Method.
  4. A surgeon is able to perform 3 to 4 FUT Methods in the same amount of time it takes him to perform just 1 FUE hence the hair transplant cost factor became an issue and the surgeons were no longer advocating the New Method.
  5. Most patients would need to undergo multiple FUE methods at a double or triple the cost compared to just one or two FUT method which already cost less, in order to achieve desired restoration hence most patients could not afford the New Method anyway. MANUAL FUE became known as the Method for the Rich and Famous, not the option for the average Joe or Jane.

 Another factor which turned many patients away from the Manual FUE was the percentage of wasted grafts. The fact is, grafts once removed from the body, CANNOT survive outside of the body for more than approximately 3 to 4 hours maximum 5. Because Manual FUE takes too long to complete, the percentage of wasted grafts would range anywhere from 10%- 30%, depending on the skill of the surgeon. Procedure Length, Limitation of Grafts harvested per session, High Surgical Cost due to Technique Dependency on the Surgeon’s involvement, Percentage of Wasted grafts to a low harvested number ratio, all contributed to low popularity of the procedure among the surgeons and the patients. Most patients will require an average of 2500 to 3500 grafts for medium to complete hair restoration. Manual FUE meant that they would need to undergo approximately 3 to 4 sessions, at a double or three times higher cost compared to a lower cost/higher number of grafts per session via FUT. In the last decade since the advent of Manual FUE, surgeons started diverting the FUE enthusiasts back to FUT which continues to lead many into confusion over the long debated issue of FUE vs FUT.
